What does the field Pager item text: * do? I cant see where that text appears at all.

It doesn't appear if you're not using a DDblock theme that supports it. If you look at our Diver theme, for example, the Pager text is the brief descriptive text that appears on the right side of the pager. If you have no intention of ever using it, you can remove the field from the content type by navigating to Administer > Content Management > Content Types and clicking 'Manage fields' for the Article content type. You can then remove the pager text field there.
A few of customers switch our themes on the fly and so the idea at first was to make them as compatible with one another as possible.
